Your GP plays a central role in helping you live well with a long-term health condition, and there are Medicare-funded programs specifically designed to support you. What Counts as a Chronic Condition?
A chronic condition is generally defined as one that has lasted or is expected to last six months or more. Common chronic conditions managed in general practice include:
- Type 2 diabetes
- Heart disease and hypertension
- Chronic obstructive pulmonary disease (COPD) and asthma
- Arthritis
- Chronic kidney disease
- Anxiety and depression
- Osteoporosis
- Thyroid conditions
If you’re living with one or more of these conditions, your GP can work with you to create a structured plan for managing your health.
Chronic Disease Management Plans
Under Medicare, patients with chronic or complex health conditions may be eligible for a GP Management Plan (GPMP) and, where relevant, a Team Care Arrangement (TCA). These are Medicare-funded programs that allow your GP to coordinate your care and refer you to other health professionals — such as physiotherapists, dietitians, diabetes educators, or exercise physiologists — with Medicare rebates applying to those services as well.
To be eligible, your condition needs to be chronic (or expected to be), and your GP needs to assess that you would benefit from structured management and team-based care. Your GP will review your situation and discuss whether a care plan is appropriate for you.
What’s Included in a Chronic Disease Care Plan?
Your GP Management Plan will include:
- A summary of your condition and treatment goals
- The actions your GP and health team will take
- What you can do yourself to manage your condition
- Details of any other health professionals involved in your care
- A schedule for review appointments
Care plans are reviewed at least annually, or more frequently if your condition changes. These reviews are bulk billed for eligible patients.
The Role of Regular GP Appointments
Beyond formal care plans, regular appointments with your GP are one of the most important tools in managing a chronic condition. These visits allow your GP to monitor your condition over time, adjust medications if needed, order relevant investigations, and catch any complications early.
Consistency matters. Seeing the same GP regularly — rather than different doctors each visit — allows your doctor to build a detailed picture of your health and notice changes that might otherwise be missed.
Medication Management
Managing medications is a significant part of living with a chronic condition, particularly for patients taking multiple medications. Your GP can conduct medication reviews, check for interactions, and coordinate with pharmacists and specialists to ensure your medication regime is safe and effective.

Supporting Your Mental Health
Chronic illness and mental health are closely linked. Living with ongoing pain, fatigue, or a condition that limits what you can do can take a significant toll on emotional wellbeing. Your GP can screen for anxiety and depression, provide a Mental Health Care Plan that gives you access to bulk-billed psychology sessions, and connect you with support services if needed.
